Enrollment Forms

These forms are only for families who have completed registration, and received an email confirmation of their spot in class from our membership coordinator.

1. fill out the registration form (use the buttons at the bottom of this page)

2. Pay your registration fee ($120) via check or CashApp. This fee will be returned to you if Creative Discoveries does not have a spot for your child in class. If you choose not to enroll, however, this fee is non-refundable.

3. Receive email confirmation of your child's spot in class

4. Fill out and return all of these enrollment forms prior to your child's first day of class
